Data Privacy Agreement

By proceeding, I understand that Project Aruga (Department of Social Welfare and Development) will collect and process personal and sensitive information for the purpose of profiling, assessment, and delivery of services for children with disabilities.

I acknowledge that:

• My data will be handled securely in compliance with the Data Privacy Act of 2012 (RA 10173).
• Information may be shared with authorized DSWD personnel and relevant partners solely for program implementation and reporting.
• I have the right to access, correct, or request the deletion of my information.
